COVID Screening

This form must be completed before your tattoo appointment.

PLEASE NOTE: This is NOT a new client contact/appointment form.
This form is for clients who already have an appointment scheduled.

Masks & Vaccines

All clients must wear either an N95, KN95, or double up on lower efficacy masks for the entire tattoo session.

 

We ask clients to follow these protocols in order to keep themselves and our artists as safe as possible

 

  • No walk-ins, all appointments must be scheduled ahead of time.

  • No additional friends or family at this time. You must arrive to your appointment alone.

  • Please wait to enter the shop until your artist is ready and lets you in.

  • Your temperature will be taken with a forehead thermometer upon arrival.

  • Everyone is required to wear a face mask at all times. If you do not have your own, one will be provided for you.

  • You will be asked to wash your hands for at least 20 seconds or us hand sanitizer upon entry.

  • 6 feet of social distancing will be observed in the lobby and throughout the shop.

  • Please arrive freshly showered and in clean clothes.

  • We are working with a reduced staff to keep exposure risks as low as possible.

  • If you develop any cold or flu like symptoms beforehand LET YOUR ARTIST KNOW and they will reschedule your appointment. All reschedules will retain their deposits.

 

If you have any symptoms, or exposure to someone with symptoms within the last two weeks, please be considerate, and reschedule your appointment—for the health and safety of all our artists, and other clients.
