COVID Screening

This form must be completed before your tattoo appointment.

PLEASE NOTE: This is NOT a new client contact/appointment form.
This form is for clients who already have an appointment scheduled.


In response to the COVID-19 pandemic, we ask clients to follow these protocols in order to keep themselves and our artists as safe as possible

  • No walk-ins, all appointments must be scheduled ahead of time.

  • No additional friends or family at this time. You must arrive to your appointment alone.

  • Please wait to enter the shop until your artist is ready and lets you in.

  • Your temperature will be taken with a forehead thermometer upon arrival.

  • Everyone is required to wear a face mask at all times. If you do not have your own, one will be provided for you.

  • You will be asked to sanitize or wash your hands for at least 20 seconds upon entry.

  • 6 feet of social distancing will be observed in the lobby and throughout the shop.

  • Please arrive freshly showered and in clean clothes.

  • We are working with a reduced staff to keep exposure risks as low as possible.

  • Please wash your hands often and refrain from touching your mask, face, or personal belongings, such as your phone, or any food/drink, during your appointment.

  • Please be prepared to pay with card or through Venmo so we handle cash as little as possible.

  • If you develop any cold or flu like symptoms beforehand LET YOUR ARTIST KNOW and they will reschedule your appointment. All reschedules will retain their deposits.
